Scientific Studies on Finasteride
“A pilot study of a bioabsorbable self-reinforced poly-L-lactic acid urethral stent combined with finasteride in the treatment of acute urinary retention from benign prostatic enlargement” by T. Isotalo et al studied the role of finasteride when given along with a urethral stent in patients who were suffering from urine retention due to benign prostatic hyperplasia. These patients were to be evaluated for any improvement in their urinary tract problem when subjected to a combined therapy. For the study to proceed, 11 men were selected who were suffering from benign prostatic hyperplasia which has caused them to suffer from urinary retention. A suprapubic catheter was inserted in them along with a bioabsorbable poly-L-lactic acid stent of the urethra that was self-reinforced and was injected cystoscopically. These patients were given a permission to urinate in a spontaneous way after 2 days of this. They were also administered with finasteride while this therapy continued. The patients were closely monitored during this time and it was seen that all of the patients began to urinate spontaneously in a duration of two weeks only. This process continued for nine more months during which clinically significant improvements were observed in these patients in terms of urine flow. This improvement, however, leads to a slight downfall when the bioabsorbable stent was absorbed in these patients after the designated time period. These patients were kept under observation in a follow-up treatment that continued for another 24 weeks and it was reported that only 3 out of 11 men required surgical intervention to treat their problem. It was concluded that the combination of bioabsorbable poly L-lactic acid urethral stent combined with finasteride can be considered as a great therapy in people suffering from BPH and are going to opt for surgery. The efficacy of this process needs to be determined by comparing it with placebo-controlled trial.
“Finasteride Treatment of Patterned Hair Loss in Normoandrogenic Postmenopausal Women” by Trüeb R.M. et al threw light upon the use of finasteride as an agent to inhibit the problem of hair loss in postmenopausal women. Finasteride has been known as an agent that inhibits an enzyme called 5 alpha reductase which is responsible for converting testosterone to dihydrotestosterone.e.its active form. Finasteride hence reduces the concentration of DHT in the serum as well as in the scalp and eventually treats the problem of male pattern hair loss in males. The use of finasteride in treating the problem of hair loss in females is not well studied, therefore, remains controversial. This study was particularly performed in order to discuss this controversial topic and for this purpose, 5 postmenopausal ladies were selected. These women had been tested in the laboratory and also via clinical assessment and had been declared as negative for hyperandrogenism. Finasteride was given to these women in two different doses i.e. 2.5 mg per day of 5 mg per day to treat the problem of hair loss. Improvements were to be checked by assessment by the patients themselves along with the investigator with the help of photographic evidence that was taken just before starting the treatment as well as at 6, 12 and 18 months of therapy. Results suggested that finasteride improved the problem of hair loss in these females and the patients themselves reported that it reduced the incidence of hair fall and speeded up the growth of new hair. These reports by the patients were further confirmed by the investigator and the photographic evidence as well and no adverse effects were observed by any of them in the patients. It was concluded that finasteride improved the problem of hair loss in postmenopausal women who have been confirmed to be not suffering from hyperandrogenism.
Description and Mechanism of Action of Finasteride
Finasteride is a medication belonging to a class of drugs known as 5 alpha reductase inhibitor. 5 alpha reductase are of two types. Finasteride is able to inhibit the production and performance of both types of these enzymes, naming type II and type III. This enzyme is used to convert the male hormone testosterone into dihydrotestosterone (DHT). DHT is useful in the enlargement and development of the prostate. As finasteride decreases the levels of 5 alpha reductase, it decreases the amount of DHT produced in the body, leading to a normal size of the enlarged prostate.
Finalo Effects for Hair Loss Pattern
Finasteride is commonly prescribed for the treatment of benign prostatic hyperplasia and hair loss. Finalo is useful in the reduction of signs of androgenic alopecia, which is due to overproduction of DHT. A proper course of finasteride is useful to remove the symptoms of both diseases within 3 to 6 months. Finasteride has been prescribed for many years and is still a drug of choice for these diseases.
Recommended Doses and Duration of Therapy
The recommended dosage of finasteride varies from sex to sex and from disease to disease. The men suffering from androgenic alopecia can take a dose of 1mg finasteride per day. the effects of finasteride will appear 3-4 months later, showing visible signs of hair growth. Hair loss in females can also be treated with finasteride but the dosage and duration of therapy vary. Women have to take 0.5mg finasteride for 6 months in order to remove the symptoms of alopecia. In the same way, the dosage of finasteride increases to 5mg for 6 months, if the patient is suffering from BPH.
Side Effects and Warnings
The side effects of finasteride are very uncommon and appear only if the medication is taken for a very large period of time ranging up to 10-12 months.
Ejaculation disorders can also appear which can be treated over a period of time All these side effects are usually self-healing and the patient gets better as the time passes. Recently, FDA has proved that finasteride is associated with the increased risk of prostate cancer.
Finasteride has no serious drug interactions but the patients taking it must always inform their physician about the medications they have been taking or have taken a few months before. Finasteride has shown some unwanted effects when given in combination with diltiazem, tetracyclines, cyclosporins, aliskiren and other commonly prescribed medications. Not only the allopathic drugs, but finasteride has also shown some unwanted effects with herbs like St. Johns wort.
